General
These terms and conditions apply when you, the customer, order products through the website lifeform.space (the “Website”). The below return and warranty terms shall not apply for products purchased from a reseller - please contact the reseller directly for any inquiries regarding such purchases.
A purchase agreement shall be deemed entered between you and Lifeform, LLC. (“Lifeform” or “us” or “we”) when you receive an order confirmation via email (which is your receipt).
Customer Service
If you have any questions or concerns regarding products, please contact our customer support at support.lifeform.space. You should always contact our customer service prior to returning any product. As part of our service, and to be able to give you the best technical support and troubleshooting process, we will save your contact information and purchase history for a period equivalent to what we consider the product’s reasonable lifespan to be.
Prices and Payment
All applicable prices for products and payment options are available on the Website.
Lifeform has the right to withhold an order until payment has been processed and approved. Prices are stated in the following currencies available on our Website: USD($), GBP(£), JPY(¥), and EUR(€); however, all sales to customers within the U.S. shall be in U.S. Dollars. Sales tax may be applied and charged at the time of order. If not, reporting and payment of sales tax shall be assumed by you as the customer.
We reserve the right to cancel any order and refund the payment before the product has been shipped.
Order Changes
Once an order has been placed and confirmed, you will not be permitted to modify the items selected. Please contact support should you have any requests.
Delivery
All orders are delivered from our warehouse in the U.S. Once your package is shipped, you will receive a shipping confirmation with the tracking information by email. Due to courier restrictions and the risk of credit card fraud, we cannot deliver to PO BOXES.
If your package arrives damaged in shipping or with missing content, please report this to support.lifeform.space within 24 hours after delivery and we will process your request. Make sure to include photo documentation and notify the courier company as well. If you receive a damaged parcel, we invite you to refuse your delivery directly and contact us immediately.
Lifeform ships free of charge for all orders. If the customer wants to make a return, they must also assume the return shipping cost.
Returns
You have the right to return your ordered product within 14 days from the day you receive it. However, you must let us know before the return period has expired by submitting a request to us at support.lifeform.space/returns/new. A valid reference to the original invoice or receipt must be provided.
Products shall be sent back within 14 days from the day you received your parcel. Shipping costs are paid by the customer on all returns. Shipping charges from our warehouse will not be refunded.
The product must be returned in the same condition as when it was received. Any returned product must be in its original container with all labels, protective stickers, and packed with care. We reserve the right to impose a processing fee or refuse to accept a return which fails to comply with the foregoing instructions.
Any refund will be made to the source of the original purchase.
Lifeform will not be held responsible in case of lost parcels in return.
Product Warranty
Lifeform warrants that products sold through the website lifeform.space shall be free from manufacturing defects in material and workmanship when delivered to you (the “Product Warranty”). The Product Warranty is limited to 12 months from the date of delivery and extends only to you as the product’s original purchaser. Proof of original receipt and purchase are required. In the event Lifeform confirms a product is defective and falls under the terms of the Product Warranty set forth in these terms and conditions, Lifeform agrees to, in its sole discretion, either repair the product, replace it with a functional equivalent product, or refund your purchase price. The foregoing shall be your sole and exclusive remedy for a defective product.
The Product Warranty does not cover, and no remedy shall be owed by Lifeform for: (a) damage from physical abuse such as dropping the unit or impact from hard objects; (b) physically damaged connectors; (c) broken components; (d) damage, deterioration, or malfunction resulting from accident, negligence, misuse, abuse, improper installation, or operation or failure to follow instructions according to the owner's manual for this product; (e) any damages due to shipment of the product (claims must be presented to the carrier); (f) repair or attempted repair by anyone other than, or approved by, Lifeform; (g) any unit which has been altered or on which the serial number has been defaced, modified, or removed; (h) normal wear and any periodic maintenance; (i) damage due to inappropriate storage; (j) deterioration due to perspiration, corrosive atmosphere, or other external causes such as extremes in temperature or humidity; (k) damages attributable to power line surge or related electrical abnormalities; (l) damage due to connecting other than certified power supplies; (m) damage due to liquid contamination and/or battery leakage; (n) damage resulting from any use other than that it was intended for; (o) product purchased as ex-demo, refurbished stock, or second-hand; (p) any aesthetic alterations due to normal usage or aging.
Spare parts delivered are considered separate products and shall not extend the initial 12-month warranty period for the original product, nor shall any intervening authorized repair.
As stated above, the Product Warranty does not apply to any product purchased other than through the Website.
We do not recommend you attempt to service the unit yourself as this will void the warranty.
Warranty Claims
If you believe your product is defective and covered by the Product Warranty, you must notify us promptly by submitting a message at support.lifeform.space/requests/new, as soon as possible after you have discovered the defect, but in no case later than two months after discovering the defect.
You may not send an item for repair without contacting Lifeform first through support.lifeform.space/requests/new.
Please provide us with your original order number, a detailed description of the issue, and video or pictures showcasing the issue. We will email you further instructions on how to proceed. All products must be sent in original packaging and packed with care. If not, we reserve the right to impose a fee or refuse to accept the repair.
Claims relating to products purchased from a reseller must be taken up with the reseller.
Waiver of Implied Warranties
The Product Warranty and associated rights and remedies set forth above are exclusive and in lieu of all other warranties, representations, conditions, rights, and remedies with respect to any product, express or implied, statutory or otherwise, and whether written or oral; all other warranties, representations, conditions, rights, and remedies, including, but not limited to, any warranty of merchantability, durability, or fitness for a particular purpose, are hereby expressly disclaimed, excluded, and waived by the purchaser to the fullest extent permitted by law.
Force Majeure
Lifeform shall not be liable for defects or delays caused by circumstances beyond our reasonable control, such as general labor disputes, natural and unavoidable catastrophes, war, fire, amendments to regulations issued by governmental authorities, issues with power mains or telephone communications, data transmissions or any other form of communication, or defects or delays in deliveries by sub-contractors.
Limitation of Liability
Notwithstanding any term to the contrary herein, under no circumstances shall Lifeform or any parent company or affiliate thereof be liable for any indirect, incidental, or consequential damages, whether arising out of breach of contract, negligence, tort, or otherwise.

Applicable Law
Subject to non-waivable consumer rights under applicable law, any dispute or claim relating to a Lifeform product shall be subject to the conditions and limitations as set forth under these terms and conditions, and shall further be governed by the laws of the state of Illinois.
Arbitration
Unless you notify Lifeform in writing via https://support.lifeform.space/requests/new within 10 days of placing your order expressly opting out of mandatory arbitration for any future dispute, then the following terms shall apply: Any controversy or claim arising in connection with a Lifeform product or otherwise under these terms and conditions shall be settled by arbitration in accordance with the Commercial Arbitration Rules of the American Arbitration Association, and judgment upon the award rendered by the arbitrator may be entered into any court having jurisdiction thereof. The arbitrator shall issue a written opinion explaining the reasons for the award. The arbitrator shall award the prevailing party reimbursement of reasonable attorney’s fees. The arbitration shall be conducted in Cook, Lake, or DuPage County, Illinois.
Changes to This Document
Lifeform reserves the right to change and make additions to these terms and conditions at any time. You are bound by the terms and conditions valid at the time of your purchase. Changes and additions to these terms and conditions are valid for all purchases made after publication at lifeform.space.
